Service Ticket for Equipment Repairs
Pro Pack Solutions repairs many types of tabletop packaging equipment, including gummed tape dispensers, label dispensers, twist tie machines, coders, and stencil machines. Use the service ticket form below when sending a machine in for repair.

Complete the PDF form on your computer, then print it and place it inside the box with your machine. You may also email the completed form to us, but a printed copy should still be included with the shipment so our technicians have your contact information when the machine arrives.

We are located in the Atlanta metro area. Local customers are welcome to contact us about dropping equipment off in person.
Before Shipping Your Machine
- Include the completed service ticket inside the carton.
- Include all related machine parts and components.
- Pack the machine securely with at least 2 inches of protective clearance on all sides, top, and bottom.
- Prepay shipping and insure the package for the value of the machine.
- Shipping charges to and from our shop are the customer’s responsibility.
Repairs are quoted as complete repairs, including replacement of missing parts if needed.
Warranty Repairs
If your machine may be under warranty, include a copy of the packing slip or invoice. If the machine was not purchased from Pro Pack Solutions, contact us before shipping for warranty consideration. In many cases, manufacturers require warranty claims to be handled through the original selling distributor, and some manufacturers perform warranty service through their own channels.
Repair Approval and Storage Policy
Machines will be held for up to 30 days awaiting repair approval. Equipment left beyond 30 days without approval may be disposed of.
